Starting third year medical school

This is the second month of my hospital rotations. Last month I was working in a family care clinic. It was a good place for me to start on as they simply threw me in with the patients and I got a lot of practice taking histories and physicals. One of the docs is scheduled as the attending physician each day that is in charge of the med students and residents. As a student, I would go meet my patient, gather information about why they are there, obtain other history, run through a review of systems to ferret out any other current problems, and a physical exam that seems appropriate to their visit. Then, armed with all the info I could think of asking, I would excuse myself and go present to my attending physician. Sometimes you have to wait in line but there were usually only a few of us and the residents require less of the attending's time anyway. My attending would then, without fail, ask a bunch of questions that I had not thought about. Then they would ask me what I think was going on and what should be done and finally they would go in the room to meet the patient with me to ask those questions I didn't think to ask and to repeat some of the physical exam themselves. They'd discuss whatever needed to be done with the patient and we would exit to go print out whatever prescriptions, lab orders, referrals, etc that were needed. I can't sign anything as a med student so the attending has to sign all those papers. Finally, I'd pop back in, give the patient the papers and sometimes some drug samples, and conclude the visit. Then I had the joy of charting the visit! Ugh, since I'm still figuring this all out, not only did the visits themselves almost invariably take at least an hour but I took another hour to write it all down. Usually I had my next patient scheduled and had to wait till the end of the day to finish charting. The charts I had to submit to my attending so they could check them over and sign them off. I was usually brain fried by the end of the day, which often went from 7am to 6pm, but I gained a lot of practical experience which I am glad for.

This month, I am working with a surgical urologist who has his office right by my hospital. He sees patients on Monday and Tuesday at his practice. The rest of the week is a mix of office visits with surgical procedures at the three hospitals in the area. My base hospital is actually made up of two hospital campuses, Mercy and General, that partnered together to form a single hospital system. Between the two campuses on the freeway is another hospital called Hackley. My doc sees patients at all three of them and he is a tall man so I am usually jogging to keep up with him in the hallways because I would most certainly get lost if I wasn't with him! It has been already a very interesting rotation working with my doc. I learned how to change catheters; I got to participate in a prostate biopsy and an inguinal hernia repair. I've watched some bladder scoping (thankfully they usually have a camera so I can see what's happening on screen), putting in ureter stents between the bladder and kidney and putting in a suprapubic catheter. A suprapubic catheter goes into a hole that the doc makes from below the belly button into the bladder to help drain if the usual catheter isn't doing the job. I even saw the process of breaking up kidney stones with shock wave treatment and how they put in the radioactive seeds for prostate cancer treatment. And this was all in the first week!

Well that kind of summarizes my third year experience so far. We have a lot of 7am lectures so I am often going to bed at 10pm. There are also a number of lunch meetings, afternoon lectures on Thursdays, and morning lectures on Fridays. Some of those meetings are journal clubs so we have articles to read beforehand and they expect us to be reading about everything we're seeing anyway. They keep us pretty busy but I've had the chance to go fishing a few times when we have nice weather!

Update continued...

The next morning, Wednesday, Meg and I set out to drive to New York where we'd be taking a flight out to Paris. This went, of course, not without a catch. I had taken my car in for an oil change and tune up in preparation for the trip a week prior. They suggested the power steering and transmission be flushed as well so I had them do that. Later we realized they hadn't changed the oil we had originally gone to have done, however, exams and packing put it out of my mind until just as we were finally ready to leave for NY. We looked up the earliest oil change place that would be open and stopped there on our way out of town. Lo and behold, as we waited the guy doing the oil change came in holding a palette with a sample of my power steering fluid (black) next to a sample of what it should look like (pink). What gives?! I just had that flushed less than a week ago! When we get back into town I will have to go ask the first car place about this but it seems awfully suspicious to me.

Besides the catch with which we started our trip, the rest of the way was a rather uneventful but enjoyable ride listening to audiobooks. Those things are the friend of any long car ride!

We actually didn't go straight to New York but stayed with Meg's uncle's family in New Jersey. They dropped us off in NY the next day so we could catch a bus to the airport. It took us an hour wait just to get into the city! Couple the traffic with how expensive everything is and I am not sure why anyone would want to live there. I think I am a medium town kind of guy. Phoenix may be big but at least it isn't crowded like NYC!

I have a definite allergy to dust or whatever is related to dust, like dust mites. As soon as we sat down on the plane I couldn't stop coughing because of a tickle in my throat. At first I thought it was the heavy cologne from one of our fellow passengers but finally zeroed in on the airplane blanket I had already draped on myself. As sooon as I took it off, the coughing ceased! That's happened to me before trying to sleep on old carpet and I think it is the same reason.

Update (boards, school, moving, travel)

Okay, so I admit I haven't been posting much on my blog for the last few months - even though things have been happening that are blog-worthy!

To start with, I spent the entire month of June sitting in a coffee shop studying for Step 1 of the D.O. and M.D. board exams (COMLEX and USMLE respectively), which represent the cumulative knowledge I supposedly gained during my two years in medical school. I took my USMLE exam on July 8th and the COMLEX on the 14th of July -I passed USMLE!!! However, this being the first year that COMLEX was administered electronically, they will not be getting us our results until the end of August so that they can have a good enough sampling of exams to calculate percentile scores. The rumor is that people who took their COMLEX at the end of July were scoring much higher than those who took theirs in early July (where does that put me?) - whether they simply had more time to study or because there was too much communication with those who had just taken the exam, I don't know. Sounds like they have a very complex system to score these things.

Because they gave us a month to study for said board exams, our summer semester was VERY short (although not short enough most of us would say). Our class is the last of our kind, as starting with next year's class, they now commence medical school during the summer and are done with the sitting for boards - none of this mucking about for an extra month afterward! ;o)
Nonetheless, even with full blow senioritis, I managed to pass my short summer classes. It helps that I had already learned some of the material seeing as how we had to study it for the boards we already took... that's a definite plus to the next generation of med students after us at our school; they will have finished all their courses and will have covered all that material BEFORE boards.

We took our last exam and I turned in my school mailbox key last Tuesday. Then I went back to my apartment to hurriedly pack up all my stuff (during the hottest day of the year so far in Lansing, much like their winter days are not) and move so that I could check out before the rental office closed for the night. We piled boxes of my stuff in Meg's parent's garage as they were kind enough to let me keep it there for a few weeks and so ended this two-year chapter in my life.

Spring break in Death Valley

I got to enjoy a full week of camping in Death Valley for our Spring Break with some of my friends at school. We had a great time! You wouldn't think it from the name, but Death Valley National Park is really beautiful and has a lot to offer in terms of things to see. Here's some pics from the trip.

Long lived bureaucracies

Stick with me, this is great. What you may think is ancient history still casts its shadow on the present!

The US standard railroad gauge (distance between the rails) is 4 feet, 8.5 inches. That's an exceedingly odd number. Why was that gauge used? Because that's the way they built them in England, and English expatriates built the US Railroads. Why did the English build them like that? Because the first rail lines were built by the same people who built the pre-railroad tramways, and that's the gauge they used. Why did "they" use that gauge then? Because the people who built the tramways used the same jigs and tools that they used for building wagons, which used that wheel spacing. Okay! Why did the wagons have that particular odd wheel spacing? Well, if they tried to use any other spacing, the wagon wheels would break on some of the old, long distance roads in England, because that's the spacing of the wheel ruts. So who built those old rutted roads? Imperial Rome built the first long distance roads in Europe (and England) for their legions. The roads have been used ever since. And the ruts in the roads? Roman war chariots formed the initial ruts, which everyone else had to match for fear of destroying their wagon wheels. Since the chariots were made for Imperial Rome, they were all alike in the matter of wheel spacing. The United States standard railroad gauge of 4 feet, 8.5 inches is derived from the original specifications for an Imperial Roman war chariot. And bureaucracies live forever.
So the next time you are handed a spec and told we have always done it that way and wonder what horse's ass came up with that, you may be exactly right, because the Imperial Roman war chariots were made just wide enough to accommodate the back ends of two war horses. Now the twist to the story... When you see a Space Shuttle sitting on its launch pad, there are two big booster rockets attached to the sides of the main fuel tank. These are solid rocket boosters, or SRBs. The SRBs are made by Thiokol at their factory in Utah. The engineers who designed the SRBs would have preferred to make them a bit fatter, but the SRBs had to be shipped by train from the factory to the launch site. The railroad line from the factory happens to run through a tunnel in the mountains. The SRBs had to fit through that tunnel. The tunnel is slightly wider than the railroad track, and the railroad track, as you now know, is about as wide as two horses' behinds. So, a major Space Shuttle design feature of what is arguably the world's most advanced transportation system was determined over two thousand years ago by the width of a horse's ass.
